Transitions program/staff encourage and support individuals as they explore new opportunities which promote personal growth. They promote each individual’s choices – striving to provide needed supports, balancing risk with clinical judgment to ensure safety. Vocational staff offer assistance with job placement, supported employment, and job coaching.
Services offered through the Transitions program include community inclusions activities, which are based on consumer interest and supported by Transitions Program Staff. In addition, the program may support vocational placement, volunteer placement, educational experiences and leisure recreational experiences. Personal care, behavioral supports in a culture of gentleness, transportation, speech therapy, occupational therapy , nutrition services may be a part of the person’s daily schedule. Activities may be individual or group based experiences.
The Transitions Program is operated and supported through Community Services for the Developmentally Disabled. The Transitions Program provides assistance to adults with developmental disabilities in expanding life options through inclusion into community live. These expectations are achieved through skill building activities in program and community settings. The services are provided through skill building activities and are developed as part of the Person Centered Planning process. Goals and objectives are developed to address increasing independence and skills in identified areas. Activity development is built around new experiences, skill development, community integration, interpersonal skills and increased personal growth.
Transitions provides services in site based and community settings. There are program sites throughout Clinton, Eaton and Ingham counties. Program sites differ in size and support consumers with differing levels of care.
The larger sites are located in Holt (Transitions-South) and Lansing (Transitions-North) and serve individuals with the most serious needs.
Smaller sites for consumers with more independence are located in St. Johns, Lansing, Charlotte, Grand Ledge and Mason.
Transitions-North provides opportunities for work training in a workshop and consumers throughout the CMHA-CEI area may also attend Peckham Vocational Industries for work training.
Transitions provides assistance with community job placement and supports through a program housed in Lansing—Transitions Employment Solutions.
