    Community Service for Developmentally Disabled
    The mission of CSDD is to promote and support ongoing choices and opportunities for children and adults and their families to be full and equal citizens in the community.

Special needs funds can be made available to eligible consumers of CSDD services who live with their families to purchase specialized equipment for the consumer. Availability is determined based on medical necessity and the amount or resources available to provide equitable access.

Special needs funds are limited, and not intended to meet all of an individual’s needs, as some may be better met by community and natural supports.




Items that are medically necessary must assist the consumer in one of three (3) areas;

  1. Increase community inclusion and participation
  2. Increase independence or
  3. Increase productivity

To submit a request for special needs funds, the following must be included:

  1. A Prescription from the doctor with diagnosis included
  2. A written request from a therapist with a goal and plan for the item. (Or parent with what the intended goal and plan is for the item.)
  3. The order form, product information and page describing a catalog item should also accompany the request.
  4. The maximum expenditure for special needs is $300.00

For consumers who do not have Medicaid, access is very limited. On an individual basis requests that meet the above criteria of medical necessity standards and have the above four items included, can be considered if there is also:

  1. A written denial from private insurance.
  2. Documentation is provided by family that they lack the financial means to pay for the item.
  3. Other charitable organizations have been exhausted or are not available
  4. The family’s monthly co-pay has been expended or will be applied toward the special need request

All Requests are sent to the coordinator of the Family Support Program for review.
