The mission of CSDD is to promote and support ongoing choices and opportunities for children and adults and their families to be full and equal citizens in the community.

Healthcare integration seeks to enhance access to behavioral health. Primary Care Behavioral Health is achieved through partnering with primary care providers to have behavioral health services integrated into the service offered to patients at their primary care office. Providing behavioral healthcare in primary care settings offers an opportunity to address both health behavior as well as behavioral health concerns. This model seeks to promote functional restoration for patients while they are accessing medical care. This model utilizes patient activation to support patients in partnering with their healthcare provider to achieve their healthcare goals. Behavioral health staff address common behavioral health issues such as depression and anxiety, as well as health behaviors which may be impacting their health such as medication compliance, nutrition and activity level.
