To obtain a copy of your Clinical Records, please follow the below instructions to complete a Release of Information(ROI) (Located on the right). A sample Release is available (on the right) if needed.

Instructions:
Please fill in all the blank items on this release with the exception of the client number. If you are not the consumer/client, please attach a copy of proof of guardianship, representative of the estate, custody decree, etc., if we do not have a current copy on file. Please check all of the boxes that apply to your request and submit the ORIGINAL release to the Central Clinical Records location in Lansing.

If any items on the release are NOT filled out correctly or left blank, the release will not be accepted and will be sent back to the responsible party for further completion.

Time Line for Receipt of ROI:
As required by law, this facility has 30 days to fulfill the request for information. If the requested records are not stored or accessible on site, Federal and State laws allows CMHA-CEI up to an additional 30 days to process a request for information.

Charges:
Charges may apply under certain circumstances. Please note that an invoice will be sent to you prior to the records being sent. Once payment is received, record copies will be sent US mail or through secure email. (MCL 333.26269) 

Initial Search Fee: $25.38 
First 20 Pages: $1.27 each
Pages 21 - 50 $0.63 each
Pages 51 and over: $0.25 each
Postage Cost (varies due to size)

Please note: (Per CMHA-CEI Policy & Procedure 3.2.11) Adult Mental Health Treatment case records will be retained until the last date of active service plus ten years. Children's Mental Health Treatment case records will be retained until the client is seven years past the age of majority and last date of active service in any CMHA-CEI program is ten plus years.
